Synopsis.
Name
The Paper Trail. Failure is punished not by whether the aim was reached but by whether the process was followed, so every hand learns to work for the record instead of the result: the doctor orders the test that shields the chart and not the patient, the clerk sends the email that proves the concern was raised, the manager collects the sign-off that spreads the blame ... and the paper piles up, and the effort that should have gone to the outcome goes to being covered, until the thing fails anyway and everyone, truthfully, points to their spotless file.
Arc
The bend is this: where failure is blamed by the process, the safe work is the work that documents itself, not the work that succeeds. It runs on an incentive pointed at the wrong target. When a bad outcome is met by asking who followed the rules rather than what went wrong, the rational move for each person is to become defensible: to do the act that leaves a clean record, to get the sign-off, to tick the box, whether or not it helps the aim. Defensibility, not effectiveness, is what protects you, and the two are not the same. None of it is laziness. The coverer is right that an undocumented judgement will be punished if it fails and a documented folly will not, so it documents; the auditor certifies the boxes because boxes are what it was told to check; and the manager spreads the sign-off because a decision owned alone is a decision blamed alone. So the paper accretes, the aim recedes behind the effort to be blameless, responsibility diffuses across a chain where everyone owns their box and no one owns the result, and when it fails the failure has a hundred covered hands and no author. Change what is punished, judge the honesty of the judgement and not the thickness of the file, give people the safety to own an outcome, and the work can turn back toward the aim: but the blame culture is entrenched and often serves those who run it, and the hardest part is that some of the paper is a real safeguard, so the hand that strips the defensive file can tear out, with it, the check that was actually holding something up.
Stakes
Whether work is ever done to reach the aim, or only ever to survive its failure. Every plot but the last moves the same way: toward the record, which protects the worker, and away from the outcome, which does not. The production cannot make a documented folly cost its author what an undocumented judgement costs, so the rational hand works for the file, and the file is not the aim. It can only show that no one was lazy, that covering yourself is the right move when failure is blamed by the process, that the auditor checked what it was told to and the manager spread what it was wise to spread, and that the one thing that would turn the work back toward the outcome, punishing the dishonest judgement and not the honest failure, is a change to the incentive that its keepers resist and that cannot strip the defensive paper without risking the safeguard hidden in it.
